胸水

 

在去年底的PET-CT扫描中即报告有右侧少量胸水，当时医生认为可能与浮肿相关，是化疗药的副作用之一。（参见“浮肿”篇）

尽管上半年种种努力把小腿浮肿搞消了，但胸水一直都在，近二个月更甚，已经到了影响呼吸的程度。

还影响到循环系统。在由立或坐位躺下的时候，心跳加速，脑子有充盈感，面部充血，呼吸困难。大喘几口气可以缓解。起立时倒没觉得有问题。

2023-8-18日 CT报告提到

right moderate and left loculated moderate pleural effusion with consolidation and or atelectasis of the lung bases.

而且腹腔内亦有变化，医生考虑是否有转移、或第二原发癌。

后天将抽右侧胸水，看看是否能改善呼吸、循环。同时检查脱落细胞和生化分析。

 

8-31日，麻醉下抽右侧胸水，a total of 1500 mL of yellow fluid was removed.

部分胸水送病理及化验。

细胞学检查：Rare atypical cells present. Insufficient for additional testing

Protein, Fluid（mg/dL）Value 4,100

LD, Fluid （IU/L）Value 145

Glucose, Fluid （mg/dL）Value 117

 

细胞及分类、培养：少量白细胞和红细胞。

Agree with differential cell count. No definitive microorganisms or malignant cells are seen in the material examined. Correlation with microbiology and cytology results is suggested.

 

抽水后呼吸轻松很多，咳嗽也轻了。不是血性胸水，没查到癌细胞，比预想的要好。

肿瘤医生对上述结果不置可否。

 

10-25日 再次看医生，提到呼吸急促和咳嗽，要求做胸透（chest x ray)：

报告提到：Small right pleural effusion. Compressive atelectasis of both lungs。

 

10-27日 时隔八周，再次抽右侧胸水，a total of 1000 mL of yellow fluid was removed.

Medical Cytology Interpretation

NEGATIVE FOR MALIGNANT CELLS

Mesothelial cells and few lymphocytes.

 

没有找到恶性细胞，是个好消息。不过胸水源源不断，说明右胸还是有问题。
