NYS WCB Case#G2029240: An "Exemplary" "Medical Report" from a "Treating Physician" Ahmed Elfiky

The following three images show the total three pages of a "medical report" from Dr. Ahmed Elfiky and his office at 153-19 Union Turnpike, Flushing, NY 11367, about my medical visits there on March 05 and 29, 2019, due to the serious injuries I sustained from the series of premeditated and coordinated murder attempts which have been veiled as "work accident" and subsequent medical "diagnoses and treatments". The series of murder attempts to me must have been prediatated and coordinated from the governmental level. For these who claim to be a "medical professional", a "government of truth and justice", a "system of liberty and equality", and/or a "world of free enterprenue", how about take a serious look at a specific "state terrorism" case for its specifics? This so-called "medical report" was FINALLY obtained by me on July 01, 2020, after multiple inquiries and attempts. This so-called "medical report" is an "exemplary" representation of IRRESPONSIBILITY, FABRICATION, DISTORTION, and FALSIFICATION. Many simple facts such as dates are even wrong; many so-called tests were NEVER performed by this medical office; and many of the "patient's statements" are absolutely fabricated by Dr. Ahmed Elfiky and his office. The CORRUPT New York State Workers' Compensation Board system would only continue to PRETEND to serve justice, health recovery, and financial compensation. 

More specifics about my interaction with Dr. Ahmed Elfiky and his office will be revealed, although rarely any people would care, just happened to my past posts about this injury case. Many people may overfear the U.S.A. "medical professionals" compentence and righteousness, but I would like to say, many of the ones I have interacted due to the murder-attempts injuries are human EVIL. The real human evils want to murder me, but I will not die without a hard fight against the evil system --- the U.S.A. capitalism evil system.

I asked Dr. Ahmed Elfiky and his office for 1) photocopy of the Intake Form I filled in on Mar. 05, 2019; 2) a copy of the "medical report", if existent, of my Mar. 05, 2019 visit; 3) (a) copy(ies) of any WCB forms such as C-4 this medical office filled and filed to NYS Workers' Compensation Board. "PROFESSIONALLY", the whole medical office just avoids the specific questions. They claim, just as many parties of this FRUAD, CORRUPT, MURDER and EVIL system often do, that they don't understand me at these moments. They just claim for me to pick up an envelope from their medical office.
