485 RFE 请教高手：I-693 supplemental form

8月5号的第一个RFE要求做TB test，但没有要I-693 supplemental form. 原文如下：

“Your orginal From I-693 cannot be accepted by this office because completion of your tuberculosis testing was not properly done; therefore, the form was not properly documented. You should return to the civil surgeion who first examined you (or another civil surgeon authorized by this Service) to properly complete the Form I-693 medical examination to include the required tuberculin skin test.

律师同时寄出我和我LP的reponse letters with TB test results provided后，9月2号USCIS收到LP的RFE response,她的case重新启动，而我的case仍然是on hold, waiting for response.

更奇怪的事来了，最近我和LP又收到8月27号发出（9月9日才寄出）的第二个RFE，还是要求做TB test。原文除了上面的以外，还有：

...see above...skin test. Please note that we cannnot accept copies of this form. You must submit a new I-693 and the I-693 supplemental form. Both forms must be completed originals and have original signatures from the applicant and the civil surgeon.

这次还要求I-693 supplemental form. 不知有没有人用过这个表？是这个吗？

上面留有civil surgeon签名的地方，但没有applicant签名的地方. 还是我找错表了？

另外，既然我LP的case已经重新启动，她的第二个RFE还要回吗？
