5W1H on Decorated Veteran Albert Wong's Killing and Suicide

News reports that Albert Wong was a decorated infantryman veteran from the wars in Iraq and Afghanistan and was kicked out from the PTSD program at Pathway Home at the Veterans Home of California. What is the 5W1H which had led to the freeing of other people, killing of three females (program executive director Christine Loeber,clinical director Jennifer Golick, and psychologist Jennifer Gonzalez), and subsequent suicide? This may be a very hard problem and question for some people to face directly and answer truthfully.
