阳阳的 OT/PT evaluations (22-23 months)

最近阳阳接受了两个 OT/PT 的评估。

MTU 的 OT/PT 结果:

Logan is a 22 month old boy diagnosed with monoplegic cerebral palsy, which affects his R UE. Logan is a new walker (approximately 4 months ago) who is still working on his balance and foot placement; however, he is able to ambulate independently without an assistive device as his primary means of mobility both in the home and in the community. His mother reports frequent falls, and this may be due to a lack of attention to where he is going as he was able to negotiate stairs with and without a railing and negotiate uneven surfaces when on task. This therapist recommends a home program that emphasizes bimanual tasks like tricycle riding and climbing on play structures to improve his object manipulation skills. Physical therapy is recommended at a frequency of 1 x45 minute monitoring sessions per month for 6 months. Re-evaluate in 6-months.

Logan is a happy 22-month old boy with a diagnosis of cerebral palsy, monoplegia. He presents with normal tone, ROM, and functional strength throughout B UEs. He is able to transition in/out of sit and go to stand, climb on and off the matt with supervision, crawl, and roll. His fine motor skills for play are within functional limits, as he is able to use pincer grasp to pick up bean-sized objects. He scored within the "Average" range in the Grasping subtest of the Peabody Developmental Motor Scales test. His visual-motor integration score, however, falls within the "Poor" category. As he is currently able to perform age-appropriate dressing and feeding tasks, recommending OT monitoring on monthly bases, 30 minutes each, to identify physical limits towards developmental milestones and address if they arise. His visual-motor integration skills will be monitored in particular. Mother is in agreement. 

HRC 的 OT 结果 (23-months):

Logan has made significant improvements in all areas of development at this point in time since beginning O.T. in-home and at his pre-school site once/week from 7/08 until 1/09. He is now using his right arm and right leg more readily in his daily skills, and has surpassed his age level in the cognitive, fine motor, receptive and expressive language skills.

DISCHARGED!
