1. DTP or DtaP (Diptheria, Tetanus, Pertussis, or whooping cough) is given at 2 months, 4 months, 6 months, 18 months, and 4-6 years.
2. DT (Diptheria Tetanus) is given at 14 years (9th grade exam) and every 10 years thereafter.
3. Polio is given orally or by injection at 2 months, 4 months, 18 months, and 4-6 years.
4. MMR (Measles, Mumps, Rubella) is given at 12-15 months and 4-6 years; reassessed and given is necessary at 11-12 years.
5. HiB (Haemophilis Influenzae-B) is given at 2 months, 4 months, 6 months, and 15 months.
6. TB (Tuberculosis) screening is done at 1 year, 5 years, 10 years, and 14 years if risk is present (annual TB skin testing may be required if child is in pre-school or day care or if risk is present).
7. Hepatitis B vaccine is given at birth, 2 months, and 9 months; reassessed and given if necessary at 11-12 years.
8. Varicella (Chicken Pox) vaccine is given after 1 year of age; reassess and given as necessary at 11-12 years of age.
